I’ve always had troubles with my triglycerides since I gained weight after college. When I had them tested in December, they came back at 4543 mg/dL. These results were so shockingly high that they had to do the tests twice to verify the number! In January I started researching options for lowering my triglycerides and came across intermittent fasting. I eased into it over a two-week period to get acclimated from the sugar withdrawal, along with cutting out all of the bad white foods in my diet. 1. What are the specific dangers and concerns of doing a fast without medical supervision?

Hey guys,

Can you go over the dangers of doing a 7-day distilled water-only fast without any medical supervision? I am on a very tight budget and can’t really afford to go see the doctor to monitor me. When I asked my personal doctor about fasting, he said that I should monitor my urine pH levels and that if they get too acidic to stop the fast. But he didn’t say what level that is. Would you concur with this? I know you can’t endorse doing a fast without medical supervision, but for folks who are going to do it anyway, is there any advice that you can give?

2. Is it possible to heal a food reactivity/allergy like eggs and dairy with a water-only fast?

Hey Jimmy and Megan,

I really really really want to permanently heal my reactivity to eggs where I experience itchy eyes, nose, lips, along with a runny nose. And I’ve done every friggin’ thing under the sun food-wise: supplements, keto, autoimmune protocol, Paleo, and fasting. At some point, my reaction to eggs inevitably returns and I need to give them up for a while. Have you seen people who have fasted permanently find success healing their reactivity to eggs and dairy? Success in my world means no symptoms or inflammation-related weight gain. I haven’t attempted a water-only fast yet and this is the next thing I would like to try. I realize this is probably going to be a long-term project and not something I’m going to knock out in two weeks. 3. Is it normal to experience stomach pain that disrupts sleep when fasting for more than 24 hours?

Hey guys,

I love listening to Fasting Talk and reading your book. I’ve been IF for a year now. Started with 16/8 and then have been doing 18/6 for a long time. I’ve done a few 24 hour fasts, a 48 hour fast, and most recently a 42 hour fast for the purposes of fat loss. My question for you guys is this: Is it common to get stomach pain when fasting for more than 24 hours? It's not hunger pangs, but feels like gastritis pain. In fact, the pain wakes me up at night it’s so bad. I don’t feel this pain when I’m eating and I only find relief from it while fasting when I drink spiced tea. Have you had any IDM patients deal with anything like this?

4. Does combining a ketogenic diet with intermittent fasting cause a delay in the menstrual cycle or even missing a period?

Hi Jimmy and Megan,

I have been doing IF and keto for about a month and half and I love the combination because it allows me to enjoy real food when I eat and gives me excellent mental clarity. My purpose in IF and keto is to prevent Type 2 diabetes since my A1c has been 5.8 the past couple of years. I’m already seeing my blood sugar and A1c drop, so I’m very happy with my nutritional choices.

However, I do have a concern that I’ve noticed. Before my IF and keto protocol, my periods usually come every 38 days. But since I changed my diet and added fasting, it has been 46 days since my last menstrual cycle and I still have no period. Does the combination of IF and keto cause a delay in the menstrual cycle or even missing a cycle? Keto Talk and Fasting Talk are my favorite podcasts and they’ve helped me so much. 5. Is there a degree or certification for ketogenic diets and fasting that you can recommend to guide clients nutritionally?

Hi Jimmy and Megan,

I'm a personal trainer and want to go to school to become a registered dietician. But since my desire is to promote the ketogenic diet, I’m concerned seeing so many RDs facing backlash and possibly losing their license for going "off-script.” As a personal trainer, we are supposed to educate our clients on how to eat, but all they promote is the government-recommended low-fat diet. 1. Why did I turn yellow and feel unwell at the end of a 7-day fast?
Dear Jimmy and Megan,

I recently completed my first 7-day fast and I felt GREAT with tons of energy. I would have continued, but I literally turned yellow and looked desiccated, haggard and unwell. I’ve completed a number of 4-5 day fasts in the past with much less of a problem than anything like this. 2. Does fasting raise blood pressure levels?

Hi guys,

For the last month, I have been doing the 5:2 version of fasting as a warm-up for some of the protocols you set out in your fasting book. I’m 46, not diabetic but I am overweight with a BMI of 28, and I’m hoping fasting will prevent the development of Type 2 diabetes, lead to weight loss, and re-calibrate my eating habits. So far I’ve lost 4 pounds, my clothes are looser, and I no longer feel polluted inside. Fasting days are not always easy, especially in this cold northern England climate, but I still look forward to them!

I have recently started having significantly higher blood pressure readings than usual (138/75 upon waking). I’m not completely sure why this is happening, but I did recently complete a course of prednisone for asthma and a chest infection. I also have a busy job with two lively young children. Enough said, right? But I have heard from people on a 5:2 fasting forum that fasting itself may have caused my elevated blood pressure.

Is it true that fasting leads to an increase in blood pressure levels? Thank you for the book and the podcast. I’ve learned so much from both and use them to inspire me when fasting gets hard.

4. Should I be concerned about my appetite dropping on feasting days to match my fasting days during an intermittent fast?

Hi Jimmy and Megan,

I love listening to Fasting Talk and I have a question for you guys. I’ve just started alternate day fasting and I’m fasting on 500 calories (M, W and F) and then eating normally the other 4 days. It’s going pretty well for me so far, but I’m finding that on the “normal eating” days my appetite has reduced a fair bit and I’m not wanting to eat the same amount as I did prior to beginning alternate day fasting. My concern is if I reduce the amount of food I eat to fit with my lowering appetite levels that it will begin to creep downward to where it is on my non-fasting days. Am I at risk for slowing down my metabolism doing this? As someone who has spent most of her life as a yo-yo dieter, I’m really worried about rebound weight gain from doing this.

Can you offer any tips on what I can do to prevent this from happening? I am loving alternate day fasting and really getting into the groove with it. Help me optimize the results I’ll get from it. Thanks for this great podcast…it’s at the top of my listening list!

5. Do the watery stools/diarrhea that can happen during and after a fast be an indication of and come from gut bacteria dying off?

Hi Jimmy and Megan,

I am one of those people who ends up with watery stools during and after I break a fast. I’ve spent a lot of time thinking about why. I know Jimmy mentioned that he was also puzzled as to what is coming out when nothing is going in during an extended fast. Is it possible that some of the bowel bacteria (which I understand make up a good deal of stool) are dying from not being fed? And since they are dead, they are eliminated from the body while extended fasting? It certainly smells like something died in there. I also wonder whether some of the health improvements of fasting occur because of the elimination of carb-loving bacteria and replacing them with the ones that survive better while fasting. I imagine those are the bacteria that would have thrived inside our ancient ancestors who regularly fasted.
Is there any merit to my theory? Thanks for the new podcast, I never miss it!

I made my first fasting attempt this week and made it 48 hours with my typical intense workouts (hard running on one day and swimming the next day) just to see what would happen. 4. Does pain symptom improvement while extended fasting mean the damaged cells are removed or is it simply the inflammation going down?
 
Hey guys,
 
I have been suffering with awful post nasal drip for over a year along with a sore Achilles heel and I thought my running career was over (I’m a long distance runner and keto athlete). I began fasting in January consuming water, tea, and bone broth. By the 36-hour, my post nasal drip had vanished and some tooth pain I was having had disappeared as well. By Day 5, my Achilles pain was 50% more tolerable. I felt so good on Day 7 that I continued my fast. I’m currently on Day 9 and ran 10 miles yesterday. I noticed my Achilles pain was minimal during that run and the morning stiffness I had been experiencing has vanished. Finally, my nagging tennis elbow in my right arm is now pain-free. Does this mean that the fast has perhaps eaten all the damaged cells that were causing all these symptoms? Or is it more likely that my inflammation is down significantly as a result of the fasting to allow healing?

5. Does black coffee activate enzymes in the liver that prevent someone who drinks it during a fast to experience the full benefits?

Love this podcast! Thank you for all you are doing for people all over the world! Keep up the great work. I was curious, I heard one of your fellow podcasters Dr. Rhonda Patrick (Find My Fitness podcast) state that even black coffee activates some enzymes in the liver. She noted that this means technically a fast that allows for black coffee is not a true fast. Personally, I feel I get the benefits of my 24 and 36 hour fasts even with my black coffee. My question for you is this: In light of this information from Dr. Patrick, can we still experience autophagy and other fasting benefits while drinking coffee? Or do we only get these benefits from a water-only fast?

Hello Jimmy and Megan!

I’m now 4 1/2 days into my first ever 7-day water fast. Thanks to The Complete Guide to Fasting, I was well-prepared and I managed the first two days just fine and the hunger pangs are all but gone now. I’m sleeping soundly, and my daily energy levels are sufficient enough to handle my normal activities, but I don’t feel like there’s enough gas in the tank for my normal exercise routines. I do have a few worrisome symptom that I wanted to ask you about. I’ve noticed a tight feeling in my throat starting in the morning of Day 3. It’s located around the hollow of my neck just at and above the level of my collar bone. It’s actually something I used to feel all the time, before I adopted a cleaner way of eating over a year ago and that sensation has been gone for several months. But now it’s back with a vengeance since I commenced extended fasting. It seems to be where I think my thyroid is, so of course I’m wondering if my thyroid is suffering or just detoxing and getting healthy and strong again. Any ideas about what might be going on?

Bless you for resurrecting the important tool of fasting! I had all but given up on shedding these extra 10 pounds that suddenly appeared a couple years ago. I’m thrilled about reforming my eating habits, improving my blood sugar, and EASILY eliminating wheat and artificial sweeteners, and of course the weight loss.

3. Would limiting my fasting to non-training days be best for someone like me doing Brazilian jiu jitsu?

Hey Jimmy and Megan,

About two years ago I was living a sedentary lifestyle and weighed 275 pounds at 5'9". Now I'm down to 190 pounds after a lot of hard work and not drastically changing my eating habits other than cutting out sodas, cakes, and other sugary snacks. I'm what you might call a "fatthlete" (fat athlete). I train Brazilian jiu jitsu five days a week and I'm a little worried about fasting because from what I understand about it vigorous physical activity can cause problems for people. What are your thoughts on circadian fasting or fasting only on days I don't train? Would those be better fasting options for me?

4. Is it okay to engage in a fast post-surgery? What are the advantages and disadvantages?

Hi guys,

I’m so glad I found Fasting Talk. I already listened to Keto Talk and this new podcast makes my life complete. What I want to know about is if there are any guidelines for fasting after surgery. Is there a time period when it’s not advisable to fast post-op? Or does it depend of the surgery? I had my appendix out just before Christmas and while I felt fine doing a 40-hour fast in the week afterwards, my husband got rather anxious about me doing this. I have been doing a 20-4 intermittent fast most days for the last year with occasional longer fasts. But I haven’t found any information about fasting post-surgery anywhere.

5. Can you fast consuming green vegetable juice and coconut water? How about with the Master cleanse?

I’m curious to know if you would ever recommend fasting on green vegetable juice and coconut water? Also, I’ve heard about a lot of people having success doing the Master cleanse which uses lemons and maple syrup? I just finished up my first extended fast at 67 hours thanks to the information that you and Megan share on your awesome podcast.  I’m down 3 pounds, blood sugar 63, blood ketones 6.6. I had improved blood pressure, amazing mental clarity and even ran my fastest 5K ever. Wow! I’m going to keep extended fasting as part of my routine along with regular intermittent fasting. I have a question for you guys that I haven’t heard addressed yet. Other than measuring changes in blood glucose and ketone levels, how would someone know they are improving their insulin resistance? Are there ways you quantify this in your patients at IDM?

1. What are the hormone improvements and adaptations that take place doing shorter fasts of 24-36 hours?
 
Dear Jimmy and Megan,
 
I have read Keto Clarity, The Obesity Code and The Complete Guide to Fasting, so thanks for all these great resources. In The Obesity Code, Dr. Fung writes about a lot of adaptations that happen during fasting including a rise in epinephrine. But according to the book, those adaptations do not kick in until around Day 3 or 4 of extended fasting. So what I want to know is what hormonal improvements or adaptations take place during an intermittent or alternate day fasting period of 24-36 hours?
 
Keep up the great work!  I’ve been intermittent fasting for 10 weeks and I’m down 20 pounds!

2. Why am I not getting the same benefits from fasting now that had I previously when I fasted? What can I do about it?

Many years ago, I would regularly fast twice weekly for 24 hours at a time. I experienced improved energy, weight loss, improved body composition, and lower inflammation levels. I then fell off the wagon for a few years with changes in employment and other life circumstances. But in recent months I have started fasting again and follow a ketogenic diet when I’m feasting. But I’ve noticed since coming back to fasting again that the amazing benefits that I had previously aren’t happening. Do you have any recommendations for someone like me who has seen the positive impact of fasting before and is not seeing it now? Why is this happening?

3. Which fasting protocol is best for weight loss? How do you lose weight doing IF? How long before you burn stored body fat while extended fasting?
 
Dear Jimmy and Megan,
 
Thank you for creating this podcast on fasting. My question for you is about weight loss. What is the best fasting strategy for losing weight? I’ve heard that when you fast, it takes about 24-36 hours for your glycogen stores in the liver to become completely depleted and then you begin to burn your own stored body fat for energy. If that's true, then intermittent fasting would seemingly not work for losing weight because after about 16-20 hours of IF you start eating again. So how is it possible to lose weight on IF? Thus, it seems that extended fasting would be better than IF for the purposes of weight loss. Am I right? And how long does it take before you start losing stored body fat during fasting and not just water weight?

4. What are the signs of electrolyte imbalance during a fast and what are your best tips for replenishing these electrolytes on a water-only extended fast?
 
Hi Jimmy and Megan,

 

I have engaged in extended water fasting twice—one 5-day fast and one 8-day fast. Interestingly, I experienced brain fog and tiredness throughout these fasts and I’m thinking I might be having issues with keeping my electrolytes in balance. So what are your very best tips for replenishing electrolytes during an extended water fast? And what are the signs to look for that electrolytes are out of whack?

 

I’m planning on doing an even longer fast the next time and your advice is appreciated. I took a multivitamin, fish oil, lite salt (for the sodium and potassium), and Himalayan rock sea salt. Thank you for your help!
