I’ve always had troubles with my triglycerides since I gained weight after college. When I had them tested in December, they came back at 4543 mg/dL. These results were so shockingly high that they had to do the tests twice to verify the number! In January I started researching options for lowering my triglycerides and came across intermittent fasting. I eased into it over a two-week period to get acclimated from the sugar withdrawal, along with cutting out all of the bad white foods in my diet. While I was going through this, I came The Compete Guide to Fasting audiobook. After listening, I felt confident that I was on the right track because for once the science actually made sense. So I started adding 36-hour fasts on the weekend and had my triglycerides tested again. The results came back at 748 mg/dL. I’ve also experienced a weight loss of 15 pounds. 1. What are the specific dangers and concerns of doing a fast without medical supervision?

Hey guys,

Can you go over the dangers of doing a 7-day distilled water-only fast without any medical supervision? I am on a very tight budget and can’t really afford to go see the doctor to monitor me. When I asked my personal doctor about fasting, he said that I should monitor my urine pH levels and that if they get too acidic to stop the fast. But he didn’t say what level that is. Would you concur with this? I know you can’t endorse doing a fast without medical supervision, but for folks who are going to do it anyway, is there any advice that you can give?

2. Is it possible to heal a food reactivity/allergy like eggs and dairy with a water-only fast?

Hey Jimmy and Megan,

I really really really want to permanently heal my reactivity to eggs where I experience itchy eyes, nose, lips, along with a runny nose. And I’ve done every friggin’ thing under the sun food-wise: supplements, keto, autoimmune protocol, Paleo, and fasting. At some point, my reaction to eggs inevitably returns and I need to give them up for a while. Have you seen people who have fasted permanently find success healing their reactivity to eggs and dairy? Success in my world means no symptoms or inflammation-related weight gain. I haven’t attempted a water-only fast yet and this is the next thing I would like to try. I realize this is probably going to be a long-term project and not something I’m going to knock out in two weeks. 3. Is it normal to experience stomach pain that disrupts sleep when fasting for more than 24 hours?

Hey guys,

I love listening to Fasting Talk and reading your book. I’ve been IF for a year now. Started with 16/8 and then have been doing 18/6 for a long time. I’ve done a few 24 hour fasts, a 48 hour fast, and most recently a 42 hour fast for the purposes of fat loss. My question for you guys is this: Is it common to get stomach pain when fasting for more than 24 hours? It's not hunger pangs, but feels like gastritis pain. In fact, the pain wakes me up at night it’s so bad. I don’t feel this pain when I’m eating and I only find relief from it while fasting when I drink spiced tea. Have you had any IDM patients deal with anything like this?

4. Does combining a ketogenic diet with intermittent fasting cause a delay in the menstrual cycle or even missing a period?

Hi Jimmy and Megan,

I have been doing IF and keto for about a month and half and I love the combination because it allows me to enjoy real food when I eat and gives me excellent mental clarity. My purpose in IF and keto is to prevent Type 2 diabetes since my A1c has been 5.8 the past couple of years. I’m already seeing my blood sugar and A1c drop, so I’m very happy with my nutritional choices.

However, I do have a concern that I’ve noticed. Before my IF and keto protocol, my periods usually come every 38 days. But since I changed my diet and added fasting, it has been 46 days since my last menstrual cycle and I still have no period. Does the combination of IF and keto cause a delay in the menstrual cycle or even missing a cycle? Keto Talk and Fasting Talk are my favorite podcasts and they’ve helped me so much. 5. Is there a degree or certification for ketogenic diets and fasting that you can recommend to guide clients nutritionally?

Hi Jimmy and Megan,

I'm a personal trainer and want to go to school to become a registered dietician. But since my desire is to promote the ketogenic diet, I’m concerned seeing so many RDs facing backlash and possibly losing their license for going "off-script.” As a personal trainer, we are supposed to educate our clients on how to eat, but all they promote is the government-recommended low-fat diet. I also want to promote fasting to my clients, but I’m not sure whether there is a degree or certification I could obtain to properly guide my clients. Do you have any advice for a health and fitness professional like me who wants to share about keto and fasting?

1. Why did I turn yellow and feel unwell at the end of a 7-day fast?
Dear Jimmy and Megan,

I recently completed my first 7-day fast and I felt GREAT with tons of energy. I would have continued, but I literally turned yellow and looked desiccated, haggard and unwell. I’ve completed a number of 4-5 day fasts in the past with much less of a problem than anything like this. Have you run across this in the IDM clinic and should I push through and hope it will this improve with time? I don’t have diabetes or any other underlying medical problems.

2. Does fasting raise blood pressure levels?

Hi guys,

For the last month, I have been doing the 5:2 version of fasting as a warm-up for some of the protocols you set out in your fasting book. I’m 46, not diabetic but I am overweight with a BMI of 28, and I’m hoping fasting will prevent the development of Type 2 diabetes, lead to weight loss, and re-calibrate my eating habits. So far I’ve lost 4 pounds, my clothes are looser, and I no longer feel polluted inside. Fasting days are not always easy, especially in this cold northern England climate, but I still look forward to them!

I have recently started having significantly higher blood pressure readings than usual (138/75 upon waking). I’m not completely sure why this is happening, but I did recently complete a course of prednisone for asthma and a chest infection. I also have a busy job with two lively young children. Enough said, right? But I have heard from people on a 5:2 fasting forum that fasting itself may have caused my elevated blood pressure.

Is it true that fasting leads to an increase in blood pressure levels? Thank you for the book and the podcast. I’ve learned so much from both and use them to inspire me when fasting gets hard.

3. Is there a difference between drinking distilled water and filtered water when you are on a water fast?

What’s the difference between drinking distilled water vs. filtered water on a water-only fast? Is there any benefit to using one over the other?

4. Should I be concerned about my appetite dropping on feasting days to match my fasting days during an intermittent fast?

Hi Jimmy and Megan,

I love listening to Fasting Talk and I have a question for you guys. I’ve just started alternate day fasting and I’m fasting on 500 calories (M, W and F) and then eating normally the other 4 days. It’s going pretty well for me so far, but I’m finding that on the “normal eating” days my appetite has reduced a fair bit and I’m not wanting to eat the same amount as I did prior to beginning alternate day fasting. My concern is if I reduce the amount of food I eat to fit with my lowering appetite levels that it will begin to creep downward to where it is on my non-fasting days. Am I at risk for slowing down my metabolism doing this? As someone who has spent most of her life as a yo-yo dieter, I’m really worried about rebound weight gain from doing this.

Can you offer any tips on what I can do to prevent this from happening? I am loving alternate day fasting and really getting into the groove with it. Help me optimize the results I’ll get from it. Thanks for this great podcast…it’s at the top of my listening list!

5. Do the watery stools/diarrhea that can happen during and after a fast be an indication of and come from gut bacteria dying off?

Hi Jimmy and Megan,

I am one of those people who ends up with watery stools during and after I break a fast. I’ve spent a lot of time thinking about why. I know Jimmy mentioned that he was also puzzled as to what is coming out when nothing is going in during an extended fast. Is it possible that some of the bowel bacteria (which I understand make up a good deal of stool) are dying from not being fed? And since they are dead, they are eliminated from the body while extended fasting? It certainly smells like something died in there. I also wonder whether some of the health improvements of fasting occur because of the elimination of carb-loving bacteria and replacing them with the ones that survive better while fasting. I imagine those are the bacteria that would have thrived inside our ancient ancestors who regularly fasted.
Is there any merit to my theory? Thanks for the new podcast, I never miss it!

Thanks for doing Fasting Talk. It’s been a useful, professional, and inspirational guide in building my fasting knowledge. I made my first fasting attempt this week and made it 48 hours with my typical intense workouts (hard running on one day and swimming the next day) just to see what would happen. To my surprise, I had lots of energy to power through my workouts. So my question for you guys is this: Is it a good idea to fast for two days prior to running a marathon? My goal is to efficiently burn fat for fuel during the race, so will this strategy help me?

4. Does pain symptom improvement while extended fasting mean the damaged cells are removed or is it simply the inflammation going down?
 
Hey guys,
 
I have been suffering with awful post nasal drip for over a year along with a sore Achilles heel and I thought my running career was over (I’m a long distance runner and keto athlete). I began fasting in January consuming water, tea, and bone broth. By the 36-hour, my post nasal drip had vanished and some tooth pain I was having had disappeared as well. By Day 5, my Achilles pain was 50% more tolerable. I felt so good on Day 7 that I continued my fast. I’m currently on Day 9 and ran 10 miles yesterday. I noticed my Achilles pain was minimal during that run and the morning stiffness I had been experiencing has vanished. Finally, my nagging tennis elbow in my right arm is now pain-free. Does this mean that the fast has perhaps eaten all the damaged cells that were causing all these symptoms? Or is it more likely that my inflammation is down significantly as a result of the fasting to allow healing?

5. Does black coffee activate enzymes in the liver that prevent someone who drinks it during a fast to experience the full benefits?

Love this podcast! Thank you for all you are doing for people all over the world! Keep up the great work. I was curious, I heard one of your fellow podcasters Dr. Rhonda Patrick (Find My Fitness podcast) state that even black coffee activates some enzymes in the liver. She noted that this means technically a fast that allows for black coffee is not a true fast. Personally, I feel I get the benefits of my 24 and 36 hour fasts even with my black coffee. My question for you is this: In light of this information from Dr. Patrick, can we still experience autophagy and other fasting benefits while drinking coffee? Or do we only get these benefits from a water-only fast?

1. Why do I have a tight feeling in my throat while I’m extended fasting? Is it my thyroid detoxing or something else?

Hello Jimmy and Megan!

I’m now 4 1/2 days into my first ever 7-day water fast. Thanks to The Complete Guide to Fasting, I was well-prepared and I managed the first two days just fine and the hunger pangs are all but gone now. I’m sleeping soundly, and my daily energy levels are sufficient enough to handle my normal activities, but I don’t feel like there’s enough gas in the tank for my normal exercise routines. I do have a few worrisome symptom that I wanted to ask you about. I’ve noticed a tight feeling in my throat starting in the morning of Day 3. It’s located around the hollow of my neck just at and above the level of my collar bone. It’s actually something I used to feel all the time, before I adopted a cleaner way of eating over a year ago and that sensation has been gone for several months. But now it’s back with a vengeance since I commenced extended fasting. It seems to be where I think my thyroid is, so of course I’m wondering if my thyroid is suffering or just detoxing and getting healthy and strong again. Any ideas about what might be going on?

Bless you for resurrecting the important tool of fasting! I had all but given up on shedding these extra 10 pounds that suddenly appeared a couple years ago. I’m thrilled about reforming my eating habits, improving my blood sugar, and EASILY eliminating wheat and artificial sweeteners, and of course the weight loss.

3. Would limiting my fasting to non-training days be best for someone like me doing Brazilian jiu jitsu?

About two years ago I was living a sedentary lifestyle and weighed 275 pounds at 5'9". Now I'm down to 190 pounds after a lot of hard work and not drastically changing my eating habits other than cutting out sodas, cakes, and other sugary snacks. I'm what you might call a "fatthlete" (fat athlete). I train Brazilian jiu jitsu five days a week and I'm a little worried about fasting because from what I understand about it vigorous physical activity can cause problems for people. What are your thoughts on circadian fasting or fasting only on days I don't train? Would those be better fasting options for me?

4. Is it okay to engage in a fast post-surgery? What are the advantages and disadvantages?

Hi guys,

I’m so glad I found Fasting Talk. I already listened to Keto Talk and this new podcast makes my life complete. What I want to know about is if there are any guidelines for fasting after surgery. Is there a time period when it’s not advisable to fast post-op? Or does it depend of the surgery? I had my appendix out just before Christmas and while I felt fine doing a 40-hour fast in the week afterwards, my husband got rather anxious about me doing this. I have been doing a 20-4 intermittent fast most days for the last year with occasional longer fasts. But I haven’t found any information about fasting post-surgery anywhere.

5. Can you fast consuming green vegetable juice and coconut water? How about with the Master cleanse?

1. What are the hormone improvements and adaptations that take place doing shorter fasts of 24-36 hours?
 
Dear Jimmy and Megan,
 
I have read Keto Clarity, The Obesity Code and The Complete Guide to Fasting, so thanks for all these great resources. In The Obesity Code, Dr. Fung writes about a lot of adaptations that happen during fasting including a rise in epinephrine. But according to the book, those adaptations do not kick in until around Day 3 or 4 of extended fasting. So what I want to know is what hormonal improvements or adaptations take place during an intermittent or alternate day fasting period of 24-36 hours?

2. Why am I not getting the same benefits from fasting now that had I previously when I fasted? What can I do about it?

Many years ago, I would regularly fast twice weekly for 24 hours at a time. I experienced improved energy, weight loss, improved body composition, and lower inflammation levels. I then fell off the wagon for a few years with changes in employment and other life circumstances. But in recent months I have started fasting again and follow a ketogenic diet when I’m feasting. But I’ve noticed since coming back to fasting again that the amazing benefits that I had previously aren’t happening. Do you have any recommendations for someone like me who has seen the positive impact of fasting before and is not seeing it now? Why is this happening?

3. Which fasting protocol is best for weight loss? How do you lose weight doing IF? How long before you burn stored body fat while extended fasting?
 
Dear Jimmy and Megan,
 
Thank you for creating this podcast on fasting. My question for you is about weight loss. What is the best fasting strategy for losing weight? I’ve heard that when you fast, it takes about 24-36 hours for your glycogen stores in the liver to become completely depleted and then you begin to burn your own stored body fat for energy. If that's true, then intermittent fasting would seemingly not work for losing weight because after about 16-20 hours of IF you start eating again. So how is it possible to lose weight on IF? Thus, it seems that extended fasting would be better than IF for the purposes of weight loss. Am I right? 4. What are the signs of electrolyte imbalance during a fast and what are your best tips for replenishing these electrolytes on a water-only extended fast?

I have engaged in extended water fasting twice—one 5-day fast and one 8-day fast. Interestingly, I experienced brain fog and tiredness throughout these fasts and I’m thinking I might be having issues with keeping my electrolytes in balance. So what are your very best tips for replenishing electrolytes during an extended water fast? And what are the signs to look for that electrolytes are out of whack?

 

I’m planning on doing an even longer fast the next time and your advice is appreciated. I took a multivitamin, fish oil, lite salt (for the sodium and potassium), and Himalayan rock sea salt. Thank you for your help!

2. Why did my fasting blood glucose spike on the day after an alternate day fast? Does eating this Mercola-styled meal actually enhance your fasting?

Hey guys,

I am really enjoying Fasting Talk. I love and appreciate Dr. Fung's ability to relate info in a way that anyone can understand. And, of course, I have listened to you, Jimmy, for a long time. I am loving Megan Ramos and look forward to hearing more from her on upcoming episodes. My question for you guys is this: I practice alternate day fasting and I take my blood sugar reading the morning after a fasting day. Much to my surprise, my last reading jumped 25 points! I have read that sometimes this happens when hormones such as cortisol, glucagon, and epinephrine are released causing the liver to release glucose. Incidentally, I did eat a small meal of 3 ounces of ham and 8 ounces green beans about 3-4 before bed, so possibly that is the culprit in my higher reading. Dr. Joe Marcela was the one who recommended eating a meal consisting of 500 calories or less with 20-30g protein and 8 ounces of green vegetable to enhance my fasting so I thought I’d give it a try.

3. What would be the benefits of fasting for only two days when it seems the most results coming after that?

Thanks for the awesomely informative podcast! I'm pretty familiar with keto, but I’m absolutely clueless about fasting. I have a question for you guys to address today about fasting. Several things seem to spike up during the first few days of fasting before going down again. Doesn't this mean that doing a two-day fast means you are only getting the worst part of the fasting experience? Don’t people who do this short of a fast deny themselves of seeing the ultimate benefits of seeing these things coming back down again and feeling good on the other side?

4. Why did I experience hypoglycemia in Day 2 of an extended fast? What can I do to prevent this from happening again?

Hi guys,

I've read The Complete Guide to Fasting and have been loving the Fasting Talk podcast. As a science teacher, I really appreciate how you present the information. I initiated what I wanted to be a four-day fast and maybe longer recently. I made it to the 32-hour mark before experiencing signs of hypoglycemia. I was shaky, dizzy, faint, and very weak despite my heart rate at around 100 bpm. When I checked my blood sugar, it was down to 33. Because I didn’t feel safe to drive at the moment, I decided to drink 8 ounces of pomegranate juice since that was the only thing I could find to bring my blood sugar back up. I assume it improved because the symptoms decreased a bit and I made it to work safely. I'm generally healthy except for elevated cholesterol and a bit of excess weight and I'm hoping these things will be fixed by fasting and keto. Any thoughts on what happened here and should I be concerned? If I try to do an extended fast again, do you have any tips to prevent this the next time?

Hey Megan,

I just wanted to tell you how your advice to switch it up is working for me. I'm on my second time through a 6 day cycle of 3 days fasting followed by 3 days feasting. Before I started, I was eating keto to satiety and slowly gaining weight. I was eating a lot at night, eating lots of macadamias, and drinking wine. So, I was primed for a change. My first day of a 3-day fast started with keto coffee (lots of coconut oil and a pinch of cayenne). Midday I had a mug of bone broth with a Tbsp of butter melted in it. At night I went right for salt-water with a dash of Chipotle chili pepper powder. I use Morton lite-salt so I can get some Potassium. On the second and third days of the fast I only drank black coffee and my hot salt-water concoction. At the end of the 3-day fast my fingers were getting cold but I had lost 7 pounds.

I broke the fast with a handful of nuts and then a big meal after about 30 minutes. I feasted for 3 days eating 2 meals a day within a 6-hour window for an 18-hour intermittent fast. I know I overate a couple times with plenty of butter and macadamia nuts for lots of fat—lots. I was poaching burgers in butter, eating rib eye steaks, fried fish with keto breading, and an entire jar of tartar sauce. Still, I didn't gain an ounce in the 3 days that I feasted. Yesterday was my first day back to fasting. I started the day with keto coffee again, and my salt-water concoction with 1 tbsp of butter, and a couple more of those without the butter later in the day. This morning I had lost another 3 pounds. So, that's 10 pounds in 7 days—the most weight I've ever lost in a week. Ever. Also, last night my blood sugar was the lowest I've ever recorded it at 66.

The first day of fasting is challenging, even the second time around. Last night I was dreaming of fast food. But, I got over it, and it was smooth sailing again here on Day 2. This pattern seems to really be working for me and I plan to continue on this path until it stops working. Thanks for the great advice. You guys are awesome.

I started my fast on Thursday at 7:30pm and went until 11:30pm Saturday temporarily breaking the fast with 1 Tbs of natural peanut butter and a few bites of brussels sprouts. Then I decided to fast all day Sunday throughTuesday at 7:30 PM. All in all, it was a total of five days with that little bit of food. What if anything did that food break do to my insulin levels and fasting efforts? I have three small children and can’t check my insulin levels, so any insights you can provide would be greatly appreciated.

I have read your book on fasting and it was so full of great knowledge that I am grateful for. I am currently breastfeeding, so I am not fasting right now. But I am looking forward to start fasting soon. In your book I took notice of how people with eating disorders, specifically anorexia, shouldn't be fasting and this makes a lot sense. But what about people who have bulimia and binge eating disorder? Should they also steer clear of fasting? Are there any physical or hormonal benefits to be gained for someone like this to start fasting or will this simply perpetuate the binging behavior.

4. How will I know when I am fat-adapted enough to push my fasting periods beyond intermittent fasting?

I have been doing some form of LCHF/keto since August 2016 and have lost 30 pounds. I've been following the mantra to only eat when I’m hungry which is how I started unintentionally intermittent fasting. Most days I end up doing 18:6 but just can't get past 20 hours. I hear people say to get fully fat-adapted before trying longer fasting, so how will I know when I am there?

I think I know why people say that women cannot fast. I used to be a woman who struggled with fasting but it was because I was making one major mistake that is more common among women and less common among men. Women are much more likely to under-eat by a significant amount of calories, then they overtrain in their workouts, and on top of that try to incorporate fasting into their health strategy. When you under-eat and also fast, the body thinks you are starving and I have heard Dr. Fung speak about this inducing starvation mode. Would you please underscore to your female listeners in particular that if you eat an adequate number of calories when you are feeding that fasting is not only fine but it can actually help to reset your circadian rhythm and other bodily functions? But then I became nauseous from drinking the broth a couple of days later and ditched that. A few days after that the coffee I was drinking made me nauseous, too. By Day 15, I was only drinking water and pushed my way to Day 24 before ending it. Why did I have such extreme nausea while fasting?

2. Does fasting make the symptoms of the autoimmune disease Hashimoto’s better or worse?

Hi Jimmy and Megan,

From what I read about fasting, it seems to be a potential therapy for autoimmune conditions. But thyroid specialist and neurologist Dr. Datis Kharrazian says that one of the 10 mistakes that Hashimoto’s patients make is the following:

“When blood sugar gets too low it raises the inflammatory messenger IL-6 and promotes autoimmune flare-ups. Symptoms of low blood sugar are most noticeable between meals or if you skip meals and include shakiness, blurred vision, crankiness and irritability, and loss of function. If you feel a jump in your function and energy after eating it confirms your blood sugar was low—when your blood sugar is stable the only thing you should feel after eating is not hungry. Constantly skipping breakfast and missing meals will aggravate your autoimmune response and promote autoimmune flare-ups.”

I know Dr. Fung has said there is no impact to the thyroid when fasting. But do we know what impact fasting has on Hashimoto’s? It seems the autoimmune disease is made worse by fasting, not better. I am under the care of a functional medicine doctor and have adopted all of his proposed lifestyle changes to heal my Hashimoto’s. I tried a two-day fast and saw ketones rise to 7.7 mmol and blood glucose drop to 58 before I broke the fast. It was a water-only fast with sea salt and occasional bone broth. Two days later, I didn’t feel great and developed a sore throat with excessive sleeping. Did I have a flare up of my Hashimoto’s from fasting?

I have been in nutritional ketosis for over two years and have successfully trained for and ran in two marathons over the past year in a fasted state. I have read The Complete Guide To Fasting and have undertaken my first extended fast which lasted four days. During the fast I had a recurrence of what appears to be a so-called “keto rash” which I have had on and off since I've been eating ketogenic. I am wondering if Dr. Fung has seen this in his fasting and ketogenic patients and what could be causing this. Is this something to be concerned about when fasting?

4. What is the shortest fasting time for getting the autophagic benefits of fasting and do adding in nutrients negatively impact autophagy?

Hey guys,

I was afraid of dealing with hunger and stressful work situations at the same time if I fasted. But hearing Jimmy talk about drinking kombucha and bone broth during his fast was my "aha" moment! And I have now lost 14 pounds in 6 days doing intermittent fasting for 24 hour while enjoying coffee with cream and Perrier eating only one low-carb meal a day.

But I am interested in getting the benefits of autophagy with my fasting. So I have a few questions about this:

5. Is it advisable for someone who is not overweight to engage in a fast?

Hi Jimmy and Megan,

Fasting seems to be most helpful for obesity and diabetes control. But there are so many other benefits of fasting that are extremely crucial for all of us, not just those with weight and blood sugar issues. I am in the minority and feel like a minority because I am not overweight and I have pretty much have been the same weight forever. What kind of fasting if any would be advised for someone like me who is not dealing with excess weight but would like to get the other benefits of fasting such us immune system reset? It seems to me our ancestors, who were not overweight, were fasting without any major negative impact on their health. Is fasting just for overweight people?

6. Isn’t the half-pound of body fat you lose daily while fasting simply “calories out?”

Hey Jimmy and Megan,

Dr. Fung has thoroughly debunked the calories in, calories out model numerous times. But when he talks about the two compartment model, he notes that the fact that a pound of fat contains about 3500 calories of energy. So while fasting you can lose about half a pound of body fat a day. So my question to you is this: Wouldn’t that be considered "calories out?” If so, then is the “calories out” equation different from the “calories in” one?

7. Is it okay to use a fiber supplement while engaged in fasting?

I would like to fast for 36 hours and then try to do longer fasts.

I grew up in Orlando and ate a typical diet throughout the first 25 years of my life. I did cut out soda/pop in my teens and noticed that I lost some weight without any change to my physical activity but I was still overweight with a poor body composition. I stayed up late, ate crap junk food and drank in university and grad school. My weight peaked while on holiday in Germany as a graduation trip, 235 pounds at 6'2". I noticed new stretch marks, a marked drop in my energy levels and my clothes were tight! I was starting to get concerned about my future health and what could happen given my family medical history of diabetes, high blood pressure, heart attack, and Alzheimer's.

I'm a scientist and I enjoy researching, so I went to work. With the limited nutritional knowledge I had and novice workout experience, I started reading various sources online. There are some great fitness subreddits that really helped me to understand what I needed to do. I started tracking my calories, lowered my fat intake, upped the protein and began a weight training and cardio regiment. After about 9 months of exercise and diet, my weight was about 220 pounds and I was significantly stronger in the gym, but I still had plenty of fat.

This is where my story changes, I met Megan Christie (Ramos). After we had been dating for a few months, treating every trip to see each other as a vacation and eating junk, Megan explained the IDM program to me in detail and I was blown away. I began to see that by eating tons of processed/refined carbohydrates, I was only hurting myself.

I also began fasting when I started eating an LCHF diet. Our cavemen ancestors certainly didn't get lucky and have a feast every day, so why do I need 3 square meals a day. I started out doing 24-hour fasts from dinner to dinner and was quickly surprised that it was much easier than I thought. After about a week, my body wasn't angry at me if at 1 pm I haven't eaten anything. I began to see weight fall off over the course of a few months. I had to buy almost an entirely new wardrobe because none of my clothes fit.

I'm proud to say that I've lost 70 pounds and now weigh about 165 pounds.

2. How do I deal with cold extremities that come when I start extended fasting?

I’m a 68-year old woman who has had a total hysterectomy, had my gallbladder removed in my early twenties, and am hypothyroid taking Synthroid. I do quite well fasting from 4-6 days. My question is what happens to me beginning on the first day of an extended fast—my hands and feet become extremely cold. It can't be from a lack of salt because this starts to happen right away in Day 1 of fasting. I tried consuming some homemade bone broth with salt, but it hasn’t helped.

I have lost weight successfully using alternate day fasting. I am 5'10" tall and currently weight 163 with a BMI of 23.4 and a 34" waist. On my fasting days, I eat from 0-500 calories and I eat LCHF on my non-fasting days. So what would happen if I did not eat LCHF on my non-fasting days? Would I still see results just at a slower pace or would that negate the benefits I am seeing?

3. If you only eat one meal a day for a 24-hour fast, how do you get adequate calories to prevent metabolic slowdown?

Okay, now I am confused. On a recent episode of Fasting Talk, I heard Megan says it is detrimental to your health to not get enough calories in on your feeding days. She noted that patients who only eat 500-800 calories a day are the hardest ones to help because their metabolism has been slowed down for so long. But then in last week’s episode from Low Carb Breckenridge with Dr. Fung answering questions, he said it’s okay to eat one meal a day when you intermittent fast and that he doesn’t really care about how many calories you get in that meal. There’s no way I could get over 1,000 calories in one meal. So if you are doing this six days a week then aren’t you getting to that place where Megan says it is detrimental with the low calorie intake? Please help me with this.

You make my job as a Mind-Body Nutrition Coach easier, knowing I am providing quality, relevant information for myself and my clients. My question for you guys is about eczema. From what I’ve read, it seems fasting is beneficial, but more so for doing an extended fast of more than three days. I'm not sure I want to fast for that long since I don't have any weight to lose. I'm 5'3", 123 pounds, 52 years old and very active with trail running 2x/week, Crossfit 2x/week and ballroom dancing 2-3 times per week. Would shorter or intermittent fasting reduce the eczema symptoms or do I need to suck it up and do an extended fast of a few days to get the most benefits? Or would a ketogenic diet alone help with this?

5. Are there any differences in the fasting experience consuming water vs. dry fasting?

I've read your book on fasting and love this podcast. I have been doing 7-day fasts and intermittent fasts on a regular basis. While reading your book, I saw that fasting is a practice followed by numerous religious traditions. But one element of several of these traditions, such as Ramadan, that I did not see mentioned in your book or discussed on your show is the impact of not drinking anything during the daytime of these religious fasting periods. Fasting means abstaining from food and drink from dawn to dusk. All the discussions of fasting assumes you are consuming water. But what about these dry fasts during the daytime? Is this something different about fasting with liquids vs. fasting without them?

Hi Jimmy and Megan!

You recently answered a question of mine about whether I could fast for three days on and three days off to get the autophagic results of an extended fast. I was concerned because I didn't think I could do an extended fast because of my very physical job. Well, long story short, I completed a 26-day fast running solely on water, green tea, and KetoAide (which is a blend of water, light salt for sodium and potassium, and magnesium citrate). I felt fine, had plenty of energy, needed very little sleep, watched some scary stuff come out of my body, dropped 30 pounds, and kept off 18 of it since ending the fast. I could have physically continued to fast, but it was somewhat mentally taxing more than physically—and I just lost the will to continue it. So I broke the fast with a handful of black olives and almonds and then had a nice three egg veggie omelet an hour later. Pure bliss!

HOT TOPIC: Does fasting raise or lower your cholesterol numbers?

Hey Jimmy and Megan,

Here’s a hot topic idea for your Fasting Talk podcast. How exactly does fasting impacts your cholesterol? I’ve heard you say how it lowered your cholesterol, but my doctor said fasting would raise mine. And he was right. I had my blood lipids drawn after a 5-day water-only fast as prescribed in The Complete Guide To Fasting and my numbers were wrecked as you’ll see in my results below. I was ketogenic in the before starting fasting, so this is a shock. I would love to hearing a discussion about this on your podcast. Maybe it only works for people with already high cholesterol levels?

Here are my numbers after fasting compared with keto:

Total cholesterol 255 -- previously 197
Triglycerides 169 -- previously 84
HDL 41 -- previously 69
LDL 180 -- previously 121

1. Is the fat-burning from a water fast equivalent to the fat-burning from being in a state of ketosis?

Hey Jimmy and Megan,

I have water fasted for 21 days and many shorter ones for spiritual reasons. I know what it's like to have zero hunger after 3 days of no food. I have plenty of energy to walk and lift light things, but I’m not ready for eight hours of hard labor during a water fast. When I’m in nutritional ketosis, I also have no hunger but I’m able to do heavy lifting and do a full eight hours of hard labor work without trouble. I'm guessing the body fat burns better when we eat fat and protein on eating days than when we water fast. But we have plenty of stored body fat to fuel us when we fast. So my question is this: why can't I do my normal routine that includes a very physically demanding job on just a water fast and have the same amount of energy as I experience in nutritional ketosis? It seems they are accomplishing the same thing and should work equally. What say you?

2. Does the autophagy that fasting produces cause any pain? And can that process prevent weight loss?

Hi Jimmy and Megan,

I’m currently in Day 7 of my fast and for the past three days my lower body, including my calves, quads, glutes, hips, and lower back, has been so sore that it has prevented me from getting adequate sleep. Full disclosure, these areas in particular are stressed in my daily life already. But fasting seems to make it worse. Could this pain be associated with the autophagy that is happening while I’m fasting? Could this be why I haven’t lost any weight during my fast? I’m consuming clear homemade vegetable broth made from carrots, onions, parsley, and celery over the past couple of days, so maybe there in an insulin response to consuming this. This has been a disaster for me and I’d love to hear your perspective on what might be going on. Thanks for considering my questions for your show.

5. Can a gastric sleeve making longer fasting contraindicative?

Hi Jimmy and Megan,

I had a gastric sleeve put on three years ago and I have been LCHF off and on for most of that time shifting to keto three months ago. I have been playing around with intermittent fasting since 2015 but decided to give an extended fast a go in 2017. My goal was to fast for as long as I could, but by Day 4 I started to feel nauseous and threw up after taking my salt in the morning. By Day 5 it got worse and I broke the fast. I actually felt so horrible that I called my doctor who recommended taking pain medications and rest. My blood pressure got very high during the fast and I felt so uncomfortable from this experience. Within a few days I felt better and was back to normal again. I don’t know why I reacted so badly, but was wondering if my gastric sleeve was the reason why my extended fasting attempt was an abysmal failure. Have you seen any patients in my situation who have had a similar experience? I really want to do extended fasts to experience the benefits that come from doing it.

I love the Fasting Talk podcast. I have been keto for over two years losing 70 pounds with stellar blood work and I have dabbled in 24-hour fasts. But I have reservations about trying a 3-day fast. My biggest concern is about the social backlash I think I’ll experience. I have never had an eating disorder but my family and friends cannot (even after all this time) fathom that I’m cutting sugar and grains out of my diet—much less doing a fast! They were so resistant to my ketogenic lifestyle describing it as an eating disorder that the thought of fasting around them intimidates me.  What do you suggest?

2. Do your senses become more heightened by Day 4 of an extended fast and why?

I've noticed that when I fast for more than 3 days, my senses seem to become more heightened. Is there any explanation for this or similar experiences reported by other people who are fasting? Thank you and keep up the fantastic work!

3. Is adding fat such as MCT oil to my tea going to stoke an insulin response during my fast?

I'm trying to use an eating plan Megan posted where she broke her fast with bone broth, salad with little or no protein, and green tea for lunch and then a small serving of meat with cooked non-starchy vegetables for dinner. My question stems from something Dr. Fung said about fat. Will having tea with MCT oil in the morning mess up my fast and produce an insulin response? I’m asking because obviously I don’t need insulin to not be released when I’m not eating.

4. How can you lower the tension that comes from fasting?

Hey guys, I’ve read The Complete Guide To Fasting and have watched a lot of Dr. Fung’s videos. I feel an increased amount of tension when I’m trying to fast that it depletes me of my willpower so much that I give up on it. I’ve suffered from varying degrees of clinical anxiety most of my life and being in ketosis is the closest to calm I’ve ever felt. I know that fasting would be extremely useful for controlling my weight and my anxiety, but I’m guessing the rise in adrenaline and cortisol are what make me feel so tense. Do you have any tricks for alleviating this feeling? I really want to keep fasting.

5. Does extended fasting lead to insomnia?

Hi Jimmy and Megan,

I'm on a 7-day fast (currently on Day 5) and I've found that over the last 3 nights I've had trouble sleeping. Is insomnia a common thing during fasting? If so, does it fade over time or the more you fast? I hope to move into an intermittent fasting protocol for long-term use and don't want to spend a couple of days per week with insomnia.

I’m not having any withdrawals from caffeine or anything else that would be keeping me awake. And I’ve been in a state of ketosis for over six months, so I’m already in fat-burning mode. My blood ketones were 3.8 mmol/L and blood glucose was 3.4 mmol/L (61 mg/dL) this afternoon and I feel great during the day without cravings or hunger pangs. Any help you can provide on this would be appreciated.

1. Is eating 500-1000 calories on days I intermittent fast going to lower my basal metabolic rate?

I find that when I intermittent fast for at least 18-24 hours, I usually only eat 500-1000 calories on those days. I eat about 46 grams of protein between one or two meals and less than 25 grams of carbs all from green leafy vegetables. The rest of my calories come from fat like butter, MCT oil, coconut oil, rendered bacon fat and olive oil. I only eat clean and avoid nuts, nut flour, artificial sweeteners, and all processed foods, and only eat 100% grass-fed and pastured meats, poultry, pork and eggs and wild fish. My blood glucose is excellent and I test my blood ketones regularly with readings always between 1.0-2.0 mmol.

Should I add more of something (probably fat) to bring up my calories on days I intermittent fast so as not to lower my basal metabolic rate? Is it a problem to only get 500-1000 on these days I do IF? I'm just not that hungry but could eat more if I had to. Thank you in advance for your help and thank you for all you guys do everyday!

2. Should I be concerned about developing gallstones while intermittent fasting?

I'm one of the genetically unlucky people whose blood sugar spikes on anything—even eating low-carb. I've lived a low-carb, gluten-free, sugar-free, Paleo, Specific Carbohydrate, and GAPS diet for many years but still have consistently high fasting sugars (6.5-7 mmol or 115-125 mg/dL) and am so insulin resistant with abdominal obesity rearing its ugly head. I've been a Jimmy Moore fan for ages but recently found Dr. Fung’s work and love what he’s doing because it makes so much sense.

I'm now eating within a 9-hour window but have in the past had acute gallstones on very low-calorie, rapid weight loss plans. Needless to say, I’m nervous about doing this or going longer in my fasting because of these fears. My health takes a big dive if I go to extremes like fasting with my thyroid issues (Hashimoto’s) and liver problems (extreme Gilbert's disease). Are people like me just not suited for fasting? And if fasting is helpful for me, then what would you recommend to avoid these gallstone problems?

3. Why am I not showing ketone production after four days of fasting?

I have been fasting for 96 consecutive hours and my Ketonix breath ketone meter is registering little to no ketones. After 96 hours, I must have depleted the glucose in my system by now. In my mind I have to be producing ketones but it’s not showing up on this meter. What could be going on here?

By the way, I’ve lost 39 pounds and 2 inches off my waist since September 15, 2016. I did enjoy three days of holiday eating with my family and fell out of ketosis after the third day. But I’ve been implementing fasting ever since and cannot get the Ketonix to register any ketones. Thanks in advance if you can offer any insights.

I have been on a ketogenic diet since April 2015 and decided to do a water-only fast in December 2016 to calibrate my new Ketonix breath ketone device. A 24-hour water fast is necessary to determine the low border and a 96 hour fast is necessary for the high border of my personal ketosis. I felt so good after four days that I decided to keep it going. Today is Day 6 and I’ve been having some problems since Day 3, including a general feeling of cold, pain in my calves, brain fog, and more. Is this normal? How can I resolve these issues and should I be concerned? Should I be supplementing my extended fast with salt, bone broth, and iodine?

5. Is fasting a better option than the HCG diet to produce weight loss?

Is fasting similar do doing the HCG diet? I have done five rounds of it over the past years to lose weight that otherwise just doesn’t come off, but each time the weight came back on faster. And now not only can I not lose weight, I am continuing to gain and there doesn't seem to be an end in sight. I suspect HCG slowed my metabolism because of the low-calorie intake putting my body is survival mode holding on to fat. Is this what is happening?

After reading The Complete Guide To Fasting, it makes so much sense to me and I’m wondering if fasting would be a better option than the HCG diet. I'm at my heaviest weight ever and I'm very discouraged. Do you think fasting could help me? Please say yes.

Jimmy and Jason respond to Dr. Phinney's concerns about fasting in Episode 5:

- Labels fasting as a “fad” saying it will produce major pushback from the health authorities when people start dying suddenly
- Getting adequate fluid, potassium, and sodium with an intermittent fast up to 24 hours is benign
- After 24 hours of no calorie intake, the body starts breaking down protein from lean tissue
- By Day 3 of fasting, the rate of lean tissue loss is equivalent to 3/4 pound per day
- The data on extended fasting in THE COMPLETE GUIDE TO FASTING is misstated
- If someone fasts for three days a week, the amount of lean tissue lost is greater than the amount of lean tissue they can regain in the other four days
- Doesn’t believe that my 10-pound lean tissue loss (according to DXA) during my 28 of 31 day fast in January 2016 was water, claims it would take 40 days of eating again to recover this loss (actually a follow-up DXA two weeks later showed all of that 10-pound “lean tissue” loss was back on my body again—so he’s mistaken)
- When someone fasts for a prolonged period of time, you’re vulnerable to “refeeding syndrome” (WWII prison camp prisoners suddenly died when they were fed copious amounts of food following extended fasting)
- There’s an acute shift in minerals out of the bloodstream into the cells causing an impairment of heart function
- The Liquid Protein Diet is used as an example of sudden death from refeeding syndrome that was reported as happening because of ketones…it shut down the science on ketosis for two decades
- For some people, a 36-hour fast is fine as long as the other 5 1/2 days of eating have adequate minerals (apparent contradiction of his earlier statement)
- We need research that shows long-term, positive, sustainability for people who are fasting to reverse diabetes and reduce body fat that goes 1-2 years
- With two-week fasting or longer, the rate of protein breakdown comes down markedly. First day of fasting produces very little protein breakdown, the rate of protein breakdown peaks on the third day of fasting, by two weeks the lean tissue loss is about one-third pound lean tissue loss daily and at 28 days it’s a quarter-pound a day of lean tissue loss (another contradiction of earlier statements)
- If you only look at urea, then the lean tissue loss seems to be much less but ammonia, creatinine, and other byproducts all make up the lean tissue loss as well so urea doesn’t give the entire picture.
- It’s unethical to write a book telling people to start fasting on their own (and yet we mention to consult with a physician often throughout our book)
- I’m not saying fasting is wrong, but we don’t know what the truth is
- Getting adequate potassium and magnesium is critically important and you’re not getting these when you are fasting
